RN Geriatric Care Manager

We are a private, fee-for-service business offering an outstanding opportunity in San Diego County for an RN Geriatric Care Manager who has worked with older adults.
This position will start with a guarantee of 20 hours per week with the expectation that it may soon grow to a full time job soon as the clients assigned increases.
Position Responsibilities:
- Professional Geriatric Care Management
- Intakes and initial visits
- Assessments
- Family and caregiver education and training
- Caregiver supervision and training
- On-call second-level (escalation) response some weekends and weeknights
- Training and scheduled meetings at office
Position Requirements:
- Professional background in Geriatric Nursing, Nurse Case Management or related field.
- A Bachelor’s or Master’s degree in a related field is a definite plus.
- 2-4 years direct care and/or care management experience within the past 10 years.
- Knowledge of local senior resources is a plus.
